But with the right care and support system, people can thrive with diabetes.GluCare has developed a new model of care that provides 24/7 coverage by tracking patient physiological and medical information from wearable technology and medical devices (such as continuous glucose monitors and insulin pumps). Our proprietary Diabetes Prevention Protocols (DPP) incorporates the use of machine learning to conduct predictive algorithms to detect issues early and constantly monitor data alongside our remote care team consisting of coaches, nurses, and physicians.Job Responsibilities: ? Participate and support all initiatives involving cardiology including remote patient monitoring and all related technology platforms GluCare creates? Work with the nursing and operational teams to set and implement program policies and priorities for the cardiology and associated programs? Establish collaborative relationships with cardiology specialists for advanced services.? Conduct case management for staff with complex needs related to cardiac care.? Serve as a clinical resource for GluCare medical personnel? Work with the operational and nursing clinical leadership to conduct staff training, presentations, quality improvement activities, research, community outreach, and other special projects.? Keep abreast of health management policy statements and emerging research, technology, and protocols within the health community related to cardiology? Participate in clinical accreditation of the facility? Participate in the establishment of specific programs/clinics such as hypertension management
