SummaryReview the running-out list, critical items, out-of-stock items, generic substitutions, drug recalls and outlines course of action based on nature of item and professional judgement.Essential Responsibilities and Duties1. Coordinate with Supply Chain Management and handle borrowing, limit quantities, institute and substitute as appropriate and informs everyone accordingly.2. Review the running-out list, critical items, out-of-stock items, and outlines course of action based on nature of item and professional judgment.3. Check received items for meeting the required hospital and SFDA standards.4. Review product evaluation requests and prepares written communication as needed.5. Periodically review items that are not being used and take appropriate corrective action. Maintain liaison with the drug information pharmacist in reviewing the formulary and other related matters.6. Work with the Drug Information Center and Investigational Drugs in preparing cost saving report.7. Keeps the pharmacy staff, managers, physicians, and other health professionals informed of significant shortages, receipts, new items through appropriate communications.8. Review floor stock requests from Hospital departments and nursing units.9. Requests and facilitates the delivery of formulary/non-formulary medications for VIP/protocol patients.10. Reviews generic substitution requests according to existing policy and procedure.11. Coordinates with pharmacy areas and Supply Chain Management regarding the return of expired medications, with guarantee letters, to the Pharmaceutical Stores for replacement by the vendors in a timelymanner.12. Work with Drug Information Centre to coordinates drug recalls announced by SFDA , US FDA, MHRA, Health Canada, and manufacturers.13. Communicates with companies to request scientific support for educational activities (e.g. Symposia)and enhance medication safety (e.g. bar-coding and look-a-like, sound-a-alike).14. Contributes to the educational needs of the department.15. Participates in self and others professional development.16. Follows hospital related policies and procedures.17. Covers in other pharmacy areas when necessary as assigned.EducationPharm.D, Masters or Bachelors Degreein Pharmacy from an accredited college of Pharmacy is required.Experience RequiredFour (4) years in hospital Pharmacy experiences with Masters Degree/ Pharm.D or equivalentor six (6) years of similar requirements with Bachelors Degree is required.
