Job Description

  • Conduct initial and ongoing assessments of patients health conditions, including vital signs, medication management, wound care, and mobility status.
  • Develop individualized care plans in collaboration with patients, their families, and other healthcare professionals, considering the patients medical needs, preferences, and goals.
  • Administer medications, injections, and intravenous therapies as prescribed by physicians, ensuring accurate dosage and proper administration techniques.
  • Provide wound care and dressing changes for patients with acute or chronic wounds, ensuring proper healing and infection prevention.
  • Monitor patients health status and response to treatment, identifying any changes or complications and reporting them to the healthcare team promptly.
  • Educate patients and their families about their medical conditions, treatment plans, medication management, and self-care techniques to promote independence and improve health outcomes.
  • Assist patients in managing symptoms such as pain, nausea, fatigue, and shortness of breath, providing comfort measures and coordinating with other healthcare providers as needed.
  • Coordinate with physicians, therapists, social workers, and other healthcare professionals involved in the patients care to ensure continuity and effectiveness of treatment.
  • Advocate for patients rights and preferences, ensuring that their needs are met and their voices are heard in the healthcare decision-making process.
  • Maintain accurate and up-to-date patient records, documenting assessments, interventions, and outcomes according to regulatory standards and organizational policies.
  • Be prepared to respond to medical emergencies and provide emergency care as needed, including CPR and first aid.
  • Follow safety protocols and infection control measures to maintain a safe and hygienic environment for patients and minimize the risk of healthcare-associated infections.
  • Stay current with advances in nursing practice, attend continuing education programs, and seek opportunities for professional growth and development in home healthcare.
