Program Coordinator-Pine Grove/Lebanon ($35,000-40,000K)Department:Supportive Concepts for FamiliesLocation:Lebanon, PAIn-Home Caregivers/Program Coordinator Positions- $35,000-40,000K + $1,000 sign on bonus.We are currently seeking candidates to fill the position of Full-Time Program Coordinator in the Lebanon/Pine Grove area.MINIMUM EDUCATION REQUIREMENTS:+ High School Diploma or GEDPREFERRED EXPERIENCE+ One year of experience in human services or related field+ Knowledge of and ability to work with individuals with Mental Health Diagnoses+ Excellent communication skills and organizational skills/detail orientedRESPONSIBILITIES OF THE JOB AND ESSENTIAL ELEMENTS:Proper and accurate completion and timely submission of required paperwork as required by State and Federal Regulations, SCFFI, and HIPAA including but not limited to:On call progress notes, biweekly case notes, consumer assessments, purchase orders, census logs, fiscal forms, maintenance forms, residential change notification forms, quarterlies, IPP, medical forms, goal documentation, community life activities, lifetime medical history, monthly progress notes, weekly progress note review.Completion of all necessary DPW/HCFA annual requirement:Food Stamps(waiver)Social Services (SSI, SSA)Re-certification of needs (PAI962, Physical, OTC)Attendance to scheduled meetings to include Department meetings, provider meetings, agency and committee meetings, staff and IC meetings, consumer and family team meetings, quarterly family meetings, Community Specialist supervision, consumer quarterly meetings.Monitor overtime and hours worked.Conduct orientation for new hires and employees transferring into new programs.Community Specialist orientation.Ensure In home orientation is completed with direct care employees and forwarded to the Training Department.Participate in On Call rotation as scheduled.Weekly scheduling.Ensure all consumers are adequately always staffed.We are an Affiliate of Apis Service, Inc., an Award Winning Non-Profit Organization Offering:Many locations throughout Lebanon and Pine GroveVirtual/Online InterviewsPaid Training (Day & Nights available)Unlimited $1,000.00 Referral BONUSES for referring your friends (ask for details)Full time Benefits- Medical, dental, vision, paid time off, 401k, short term & long term disability, life insuranceMany career advancement opportunities available w/all of our affiliates!Rewards beyond measure for... ?Making a difference in the lives of people with Behavioral Health/Intellectualand/or Developmental Disabilities.*Who We Are: Supportive Concepts for Families (SCFF) is a leading provider of services to individuals with Behavioral Health/Intellectual and/or Developmental Disabilities in Pennsylvania.
