Role and Responsibilities:A.AssessmentConduct thorough head to toe assessment on each patient at home visit on the shift. For ex:Pain assessmentNutritional statusBed ridden patient for skin integrityFall assessmentConducts age specific assessment of patientsProvides age specific monitoring of patientsProvides age specific management of patients requiring special careAssesses patients/families health education needs and intervenes accordinglyCompletes re-assessment of assigned patients by utilizing nursing process to identify care needs and documents it accurately in the appropriate formB.PlanningPlans and prioritized his/her work to provide best quality care according to patients needC.ImplementationDelivers planned care (Total patient care) to the patientAdministers medication safety, monitors patients responses to medication where indicated and reports recalled incident promptlyPositioning move the patients or body part to provide comfort. Reduce the risk of skin break down, promote skin integrity and to promote healingCode Management nurse should be able to coordinate with the doctors, hospital emergency and ambulance to shift the patient immediately during life threatening situationsNeurologic Monitoring collects and analyses patient data to prevent or minimize neurologic complicationsFluid/Electrolyte managementDemonstrate safe handling and proper use of equipmentParticipates actively in patient/family educationProvides safe environment for the patients and him/herself by following infection control policies and applying safety measures for patientsMonitors and records nutritional status of all assign patients in patients chart and informs dietitian whenever requiredFollows discharge procedure of his/her assigned patients according to hospital protocol. Multidisciplinary Care Conference plans and evaluates patients care with professionals from other disciplinesD.ImplementationEvaluate results of nursing interventions on the patients and modifies nursing care planEnsures all assign patients got quality nursing care by reassessing patients health status as per requirements and before his/her shift ends.Evaluate the level of patients satisfaction at the end of his/her shiftE.DocumentationProvides timely, comprehensive and complete nursing documentation as per nursing divisions protocol/standards Salary: AED 6,500 to 7,000 per month inclusive of fixed allowances. Previous experience managing a health service is essential.
