Job Description

Location: Tripoli Governmental Hospital(TGH)Contract: 1 YearSalary range: USD 1500-2000Opening: July 5th 2022Closing: July 15th 2022PURPOSE The social worker, as an integral part of a multidisciplinary team, asses the psychosocial and social state of patients and families and provides supportive interventions to help them adjust to serious and life-threatening conditions. In addition, S/he helps ensuring coherence between the different services interacting with the individuals/communities/he provides to the patients during their stay in Tripoli Governmental Hospital screening including for Mental Health Psychosocial Support and support in admission, discharge and referral procedures. Moreover, performed intervention may include supportive counselling, grief counselling, child advocacy, crisis intervention, patient/ family advocacy. Supports the senior social worker in the Community resource networking to facilitate support in access for vulnerable groupsACCOUNTABILITIES AND RESPONSIBILITIES Assess the vulnerability of the patient and define the level of assistance under the direction of the Senior Social worker.Ensure the data collection for patient under his/her responsibility by using existing information collection tools and reporting templatesConsolidate the monthly statistic for the social worker departmentRefer patient to most appropriate institution (humanitarian organization or public entities).Support vulnerable patient in administrative process.Perform case management during hospitalizationMaintains contacts with external interlocutors under the direction of the Senior Social Worker.

  • Contributes to the implementation and monitoring of specific projects / programs within the department
  • Collaborates with members of the health team, the patient/family to assure patients psychosocial and socioeconomics needs are addressed wherever possible.
  • Coordinate?s patient discharge and continuity of care planning with respective health care team members and other NGOs or agencies
  • Helps organize meetings; advises other staff members about his/her field of activities
  • Organizes and participates in workshops/training related to access to services for vulnerable groups, inclusion for people with disabilities, integration into the community for victims of violence, and other relevant subjects
