Its teaching hospital, the Aga Khan UniversityHospital has been accredited by the prestigious Joint Commission Internationalfor achieving the highest international healthcare standards.Job Role / Responsibilities:The Division of Pharmacy is a large multisiteintegrated service with a state of the art medication management system andstrong emphasis on pharmaceutical care & clinical pharmacy practices. Theservices are centred at the Aga Khan University Hospital, Stadium Road Campus,Joint Commission International Accredited (JCIA) facility offering; Inpatientand Outpatient Satellites Pharmacies Distribution, Point of CarePharmacotherapy Rounds, Drug and Poison Information Centre (DPIC),Pharmacokinetics Consults and one year duration post Pharm .D academic programto train hospital pharmacists.You willbe responsible in ensuring safe and effective use of medications by providingmedication order verification while assessing drug utilization review, dosageregimen and drug therapy monitoring. You should work with physicians andmultidisciplinary team for therapeutic assessments and selected drugs indisease management to ensure improving patient and practice outcomes; communicatingdrug related interventions to physicians and get it rectified, analyse andrecord the impact of interventions related to patient care, patient education,and training of pharmacy staff, including:

• identifying,resolving and preventing potential and actual drug-related problems, byobtaining and evaluating medical data including patient history, laboratorydata and review patient profiles for drug interactions, allergies,contraindications, and adverse drug reactions to determine and implement optimaldrug therapy;
• communicatingand documenting drug information given to patients or patient representatives;
• supervisingand monitoring all patient drug therapy in a designated area, for costeffectiveness and communicate all possible cost saving recommendations topatients physician(s);
• maintainingand updating personal drug knowledge database to ensure accurate provision ofpharmaceutical care of general disease states as well as specialized knowledgein the assigned area of expertise;
• participatingin clinical research activities;
• preparingand ensure imparting of training as an educator in departmental trainingprogram for clinical pharmacy development as well as deliver presentations on aregular basis to the pharmacy and other health care staff;
• documentingof all the recommendations regarding drug usage and their outcomes;
• conductingdrug utilization reviews and suggests strategies for rationalizing drug usage;monitor the efficiency of such programs and suggest practical approach toimprove the outcomes;
• Facilitatingin investigation of adverse drug reaction reported to the pharmacy departmentand compile a formal report at the end of each month.

Eligibility Criteria / Requirements:

• Bachelor Degree in Pharmacy/Doctorate of Pharmacy from a recognized university;
• Post Pharm D. or post graduate academic qualifications or training in hospital pharmacy is preferred;
• 5-6 years of relevant working experience as clinical pharmacist rounds, preferably in critical care and internal medicine setting;
• Professional registration with Pharmacy Council of Pakistan.
