Job Description

Crisis Interventionist Jordan, MN Description Provides hands-on person centered support to people with varying degrees of mental illness and/or developmental disabilities. The Crisis Interventionist implements individual behavior/treatment plans created by the Crisis Program Manager to rehabilitate and transition the individual into a less restrictive living situation.Key Responsibilities:* Provides direct care under limited supervision* Provides behavior de-escalation* Implements formal/informal program and behavior objectives under limited supervision* Follows programming outlined in the Individual Abuse Prevention Plan (IAPP), Self- Monitoring Assessment (SMA), and Coordinated Services and Support Plan and Addendum (CSSP)* Monitors and maintains safety of individuals* Interacts with individuals receiving services* Monitors health care needs* Administers medication* Drives and accompanies individual to appointments* Maintains home and needs of client by cooking, cleaning, etc.* Documents detailed narratives and behavioral data daily* Administers medication and completes Medication Administration Record (MAR)* Models behavior and mentors individual to increase daily living skills* De-escalate and eliminate maladaptive behavior* Preforms RCC trained manual restraints (EUMR) during behavioral incidents* Participates in activities and social experiences within the community with individuals* Collaborates with team to write new goals or modify existing ones* Shares behavioral, health, and program concerns with supervisor* Attends and completes all required trainings, including staff meetings* Assists with transition and training for long-term placement* Teaches and practices coping mechanisms with individuals* Assists individuals with decision making skills that support transition to long-term placement *Other duties as assigned. RequirementsExperience* 5 years of experience working with individuals who have developmental/mental health disabilities required OR 4 years of post-secondary education in Human Services required* 1+ year of experience working with individuals who have high behaviors (biting, hitting, kicking) preferred* Experience with person-centered thinking and planning* Basic math skills* Basic domestic skills (cooking, cleaning, laundry)* Judgment and decision making skills to handle crisis situations and day- to- day events* Ability to interpret behavior trends.Physical Requirements*Please note that all physical requirements listed may not be required at the site; refer to site specific demands * Walking on an on-going basis* Perform manual restrains on individuals occasionally to frequently* Support individual by holding them upright up to 250lbs on occasion (site specific)* Climb stairs occasionally* Reach and grip with hands frequently* Stoop, kneel, crouch on occasion* Peripheral vision on an on-going basisEnvironmental Factors* Works with others on an on-going basis* Has public contact on occasion* Shift work usually* Behavioral incidents that may require physical interventionNon-Negotiable Hiring Criteria* Ability to write narratives in grammatically correct sentences in English* Communication skills adequate to relay information in English* Valid Driver?s License with a satisfactory record* Maintains vehicle insurance* Knowledge of person-centered thinking and planning
