1. Assemble discharged patient records in standard order.2. checks and analyzes records for accuracy, adequacy and consistency of documentation3. Assigns appropriate codes for each diagnosis, operation and procedure based on the internationalclassification of diseases and procedures.4. Prepares deficiency slips for incomplete records5. Collates generated abstracts from medical records and inputs information into the computer6. Records abstracts on discharge list7. Prepare and files out guides for the records which are given to transcription section.8. Answers request for retrieval of medical records and assist in data gathering for research, studies, and statistics.9. Account for daily discharges and performs audit as necessary10. Perform other related functions.
