Provide individualized care partnershipto patients on how to monitor their blood sugar levels and adjust their diets accordingly
Reviews patient records and meets regularly with patients to ensure they are managing and reaching their agreed goals( treatment, lifestyle, etc)
Coordinate and facilitate advocacy support groups for patients who have been diagnosed with diabetes or other endocrine disorders
Performs, and enables patients clinical assessments, such as measuring height and weight, taking blood pressure, checking feet for signs of injury or infection, and administering tests for glycated hemoglobin levels as part of the patients management of the disease process
Works with patientson how to manage their disease by following a healthy diet plan and getting regular exercise
Works with patientscontributing to their education on how to inject insulin or other medications using proper techniques to avoid infections or other complications
Coaching, teaching, mentoring patients onbasic principles of nutrition, meal planning, food selection, vitamins and minerals, and how to read food labels
